How can a homeowner's monthly mortgage bill increase when the mortgage has a fixed interest rate? Does an increase mean the lender changed the rate?

A fixed interest rate does not fix every part of the monthly mortgage bill. On a standard fully amortizing fixed-rate loan, the scheduled principal-and-interest payment can stay the same while taxes, insurance or other amounts change.
The easiest way to investigate is to separate the bill into its parts. The CFPB's payment explanation distinguishes principal and interest from the total payment. If the servicer collects money for property taxes and homeowners insurance in escrow, changes to those expenses can affect what you pay each month.
For example, suppose principal and interest are $1,500 and the escrow portion is $500, producing a $2,000 payment. If the required escrow portion becomes $650, the bill becomes $2,150 even though principal and interest remain $1,500. These are illustrative numbers, not a prediction of any homeowner's costs.
An escrow shortage can also affect the amount collected. Other explanations need a different check: a temporary payment subsidy may have ended, a fee may have been added, or the statement may contain an error. The CFPB's payment-change guide describes these possibilities.
Compare the old and new statements line by line. Look at the escrow analysis, tax bill and insurance renewal, then ask the servicer which component changed and why. Do not assume the note rate changed merely because the total did.
Our residential mortgages guide covers the underlying loan concepts. The useful habit is to budget for total housing costs, not just the principal-and-interest figure in an advertisement.
